Next Steps If You Test Positive for COVID, Are Exposed to COVID, or Have COVID Symptoms
The following form is designed to help students and employees understand what next steps to take if they have tested positive for COVID-19, been exposed (or potentially exposed) to COVID-19, or have symptoms of COVID-19. The form takes just a few minutes to complete, does not collect any personal information, and can be completed multiple times as your situation changes.
If you tested positive for COVID-19, then go directly to the COVID Reporting Form Appendix C.
